Starting this year, it will be mandatory for all middle school students participating in sports activities to undergo a sports physical examination conducted by a licensed physician. This decision has been made in accordance with the latest recommendations from sports organizations, with the primary goal of ensuring the well-being and safety of our students.

A sports physical is a comprehensive examination that evaluates a student's physical fitness, medical history, and any potential health risks associated with participating in physical activities. By obtaining a sports physical, we can identify any pre-existing conditions or limitations that might require special attention or modifications during sports participation.

We understand that this may be an additional requirement for you and your child, but please be assured that it is a necessary step in creating a safe and supportive sports environment. 

We value the commitment and dedication you and your child have shown toward our sports program, and we believe that prioritizing their health and safety is of utmost importance. We appreciate your cooperation and support as we implement this new policy.

ALL STUDENTS MUST TURN IN AN ATHLETIC FORM AND HAVE A SPORTS PHYSICAL BEFORE TAKING PART IN THE ATHLETIC PROGRAM. PLEASE SEE THE LINKED FORM ABOVE.

TRANSPORTATION

BUSES are available for all tryouts/practice days and games. Students will get off at the closest school to their house. Parents will need to pick up their child there.

Students can also be PICKED UP at Caloosa. PLEASE line up out in front of the school in the parent drop-off/pick-up loop.

TRY OUT/PRACTICE DAYS

Tuesday, Wednesday, and Thursday - 4:15 – 5:20  
Students will need to listen to announcements for the exact days of their tryouts.

ATHLETIC INSURANCE INFORMATION

1. Go to the Lee County School District website: https://www.leeschools.net/ 
2. Click on “Families” on the left side. 
3. In the Search area, type in School Accident Form
4. Click on “Student Accident Insurance (voluntary)”
5. In the upper middle of the screen it says “website and online enrollment”
6. At the top of the screen in Maroon letters it will say “Click here to purchase insurance now”.
7. Create an account as a parent and fill out the insurance form.
8. Send the confirmation number to Mr. Popik's email address ([email protected]).
